Why is my medicine suddenly not working, except the first time I try a different method of administration?

I am not sure if this is the right sub I am just so confused!

I have PCOS and take medicine for hormonal imbalance. I was on estrogen pills for 2 months. Overnight it stopped working. Now it sometimes even has the opposite effect and worsens my symptoms. Since then I have tried creams, synthetic pills, etc. Everything works really well the first time, and after that doesn't work at all. It's all a form of estrogen.

What is happening? The only possible explanation I found is some form of tolerance called tachyphylaxis, but I don't think each new method would work even once if that were the case.
