[Acne] Scared of Oral Spironolactone

Hi all, I have persistent hormonal acne. I've had it for 10 years and am 20 now. A couple months back, I found out that I have both a Pituitary Cyst and Adrenal PCOS (elevated Cortisol, DHEA-S, Androsterone), which are likely the culprit. Treatment wise, theres not much I can do. My endocrinologist wants to wait a few years and see if my cyst grows before we remove it and also told me she doesn't have any medications to lower my high adrenal hormones. She did offer oral Spironolactone (at a microdose of 12.5 mg to 25 mg due to my low bmi), and said it may help a little bit in altering the androgen conversion process. However, I regrettably have a serious medication phobia. Combined with my low blood pressure and migraine auras, I've been dragging my feet in filling the prescription. I currently use a compounded topical/cream version of Sprio, which is definitely helpful, but still get zits due to my internal hormone imbalance. I've tried Spearmint capsules before, and they really helped, but stopped due to my fear of liver damage. I'm considering b5 capsules, but I'm not sure if this will actually yield results. I'm going broke from trying so many "miracle cures" and going crazy from the low self esteem. What should I do? Does anyone have experiences taking Spiro with low BP/body mass?
