It keeps going soft

For the past three months I've been having sex with my new (and first) girlfriend, and I can extremely rarely maintain an erection to do so. Mostly I have to use a cock ring, but even then it can go soft. I haven't watched porn since January and in this almost seven-month period I masturbated I think five times (the longest streak was 83 days). At first I felt suicidal, then desperate, now I feel almost resigned. If anything, I've made some progress in determining what the causes might be:

1 Hormonal imbalance (prolactin too high, testosterone too low, estradiol too low). Haven't checked cortisol and cholesterol, but there might be problems there too.

2 Masturbating for 13 years by clenching my pelvic floor muscles. I now can't achieve a 100% erection without flexing those muscles. I'm doing kegel and reverse kegel exercises in hopes of better isolating those muscles and assuming better control over them (little progress so far).

3 Porn which has instilled unrealistic standards of what sex should be in my mind. The girlfriend loves somewhat kinky sex - luckily - but I think indulging in porn only for 13 years (and especially during my formative years) has made me feel impotent to assume the role of a man during sex. I don't even know where to begin to tackle this issue.

4 Condoms and lack of sensitivity during sex - if it was tighter, I don't think I would lose erections as easily. Maybe my penis is too small, I don't really know.

Now my girlfriend has moved away for three months for work and I have that much time to fix my ED. I'm 26, I exercise somewhat regularly (no cardio though), don't drink or smoke, I eat only healthy food and take supplements to boost libido/erections but to little avail. Can anyone help me?
