How are doctors supposed to detect pituitary tumors when they only check TSH?

My understanding is TSH is compared to e.g. FT4 or other Thyroid hormones to tell if something is imbalanced. Certain imbalances implies a tumor is present.

So isn't just checking only TSH kinda useless for this?

And also, does it really cost THAT much to just test FT4 or are doctors just being "cheap" by not checking?

Thanks
