Three differnrt opinions...derm style

I have posted before about my Recent Surgical operation underneath my left arm, and how I am having trouble underneath it even three months later

That surgery lead my plastic surgeon and my primary care doctor to believe that I have a HS.

They told me the best thing would be to see a dermatologist to get a plan of action.

The first place I went to it’s a place that I have been going to for my rosacea. But I wanted to see this place, because when I had the infected cyst, they swore up and down it wasn’t infected, and that I didn’t know what I was doing. They didn’t prescribe anything by clindamycin gel, and I ended up having a raging infection that required two surgeries.

At that time I saw the physician assistant. So I wanted to go back to see the actual doctor of the practice, and let her know what happened. And just get her opinion on the HS as well.

During my infection course, I had tried doxycycline, and ended up with a reaction. Surprisingly because I have tolerated doxycycline well before

This doctor did not even tell me what stage I was in, she immediately told me that I needed to start Humira. And she wanted to start me on an old-school anabiotic called dapsone because I was allergic to doxycycline. I asked her if I should try other things like diet modification, weight loss etc., and she said those things never really work, it’s Humira or nothing else.

I told her I wanted to think about it. That I work in the hospital. But I didn’t want any complications when I was treating Covid patients, and that I would Be slated for the Covid vaccine soon.

2nd place. My friend had gone to another dermatologist for the dermatitis she had been experiencing with her mask.

This guy was actually pretty nice. He wanted to tell me that while weight does have something to do with HS, it was not the sole reason, and that I shouldn’t be ashamed of having a HS, and I shouldn’t be ashamed of my current weight.

He gave me information about the different stages of HS, which I thought was very helpful. He listened to the fact that I didn’t really want to be on long-term antibiotics, and he suggested I try spironolactone. He said that Aldactone has been shown in some studies to help people with HS who have issues with their hormonal imbalances. He said if I also had PCOS, That the spironolactone would actually really helped my HS. But because I didn’t have PCOS he didn’t know if this would do much. But he said it wouldn’t hurt to try because normally people just have some slight dizziness, and that most people don’t developed significant amounts of hyperkalemia, In layman’s terms high potassium levels.

I have not started the spironolactone because the medication is Tetratogenic, And I want to freeze my eggs as I am in my early 40s, don’t have kids. So I want to try to freeze my eggs first, and then start the Aldactone.

This doctor said the second stage is usually the main stay is Second line antibiotics. The second line antibiotics are Clindamycin , and rifampin. And then if I wanted I would take these antibiotics for a minimum of three months. He said success rates vary.

He said insurance companies usually want people to have failed antibiotic therapy before they approve Humira. If I failed the anabiotic therapy, then I would go to Humira. He says the problem is when people do well with the antibiotic therapy, he doesn’t like to keep people on anabiotic’s for ever, so the question is win to come off the anabiotic’s.

But again, this doctor actually understood and respected the fact that I wanted to avoid antibiotics.

3rd place. I wanted a third opinion because there was another dermatology office in my town. This guy was knowledgeable about HS but I felt had no bedside manner.

He asked me if I could really loose the weight. He said patients tell him all the time they will lose weight and never do.

I asked him about some studies that show Aldactone can work for HS, and he laughed in my face. He said I have heard of no such studies, and I will not be prescribing out that down for HS. He said this is used for PCOS an acne, but I have never heard it for HS. But I have actually heard people be prescribe this for HS, so I was very upset about his attitude. He wouldn’t even listen to me. He said i had to try different antibiotics and he prescribed me clindamycin and rifampin. I told him i didn’t want long term antibiotics and he said that’s all i can offer. So he sent to my pharmacy and said i could decide how i wanted to proceed once I got to the family.

So that’s what three different derms said. So crazy how different the doctors were!
