I am a 22 year old female who has been suffering from hair thinning and hair loss for over a year now. I assumed it was heat damage at the start but lately it has been getting too bad for me to ignore, massive shedding, hair fall every time I run my hands through it etc. The density has gone down so much but its happened so gradually, still can’t believe I didn’t see a doctor sooner though.
This month I’ve taken this much more seriously and started oiling as well as consulting with two different dermatologists. The first derm diagnosed me with alopecia areata in my bald patch, and said that he can see scarring towards the back of my scalp and suspects Lichen Planopilaris which as you can imagine has been so devastating for me. He booked me in for a biopsy in both sections (as in the bald patch and towards the back of my head) to confirm these two diagnoses.
With the second derm the experience was extremely different. I have PCOS, she pointed out the mild acne on my face too and said she suspects its a hormonal imbalance causing Female Pattern Baldness and she prescribed me Spiro. When I told her about the previous diagnosis she told me that she sees 0 scarring and that what I have is classic FPB symptoms and no biopsy is needed.
I am going to see a third dermatologist this week but my question is what are your thoughts? Should I go ahead with the biopsy- it is so expensive though (I am from London and no chance getting this through NHS) and I do like that the other doctor took a different approach, taking into account my skin etc. I don’t want to do a biopsy for no reason.
It just baffles me that two qualified dermatologists can have such different opinions, and that one can see scarring and one can’t- its either there or it isn’t surely?
I’d be really grateful for anyones opinion. This has been so distressing for me, I’ve spent so much money on consultations and I really don’t want to do something unnecessarily. Thank you
