Does follicular activity on ultrasound cause pelvic pain and on left ovary for weeks?

So I went to the Emergency Room about a week ago because I was having a lot of lower abdominal pain especially on the left side, nausea, and a lot of vaginal bleeding with black watery blood, clots, and twice a month periods. I tested negative on a urine pregnancy test, and I got a pelvic ultrasound done. Well, the doctor said everything looked good on the ultrasound.

But then when I was given my medical records, it said that I have "physiological follicular activity" on my left ovary. However, the conclusion of the report said that no acute pelvic process was found.

Does follicular activity cause all these symptoms I've been having for weeks? Or is it a hormonal imbalance caused by those follicles or caused by something else?
